What else can be done with dental implants?
Patients who have no teeth in their lower jaw may not be ready for multiple implant placements. For such patients, a conventional lower denture can be enhanced with two dental implants fitted below the front section. These implants are known as overdentures.
When applied to a patient’s upper jaw, the same procedure tends to entail a higher number of implants as the bone is usually softer. Although, like conventional dentures, implant-supported overdentures are still removed for daily cleaning, the implants make them far more stable once they are back in the patient’s mouth.

Do you need to have a healthy mouth?

What other causes of bone loss are there?

In the long term, however, the gradual loss of supporting bone is caused by chewing. After many years of having dentures, patients will require a reline procedure to compensate for this problem, so the longer a patient wears dentures, the less bone will be available for dental implants.

Can dental implants preserve bone?
One of the most significant benefits of dental implants once they have been installed is that everyday functional forces stimulate the adjacent bone, and in response to this, the bone grows stronger. Of course, each patient is different, and dental implants will have limits in terms of how much work they can do before they start to fail. What can be done if a dental implant fails to work?

Will my new teeth be joined together?
Yes. When we place multiple implants in a patient’s mouth, we join them together just like a bridge supported by natural teeth would be designed. And if there are sufficient dental implants available, it is usually easier and just as effective to make several smaller sections of bridgework each supporting a few teeth.
The quality of the surrounding bone and the quantity and position of the implants will affect which option will suit each patient. When we link implant-supported teeth, they are more robust than the individual parts and are more effective at resisting the forces of normal function that attempt to loosen the screw components, posts and/or cements that secure the underlying structure to each dental implant.

Is there anything I should know before I start my treatment?
Before we begin your dental treatment, we would provide you with a summary of your treatment planning discussion, outlining your current dental situation and informing you of alternative options to dental implants if applicable. We will give you any other relevant information such as the estimated duration of the treatment, the number of dental implants required and the estimated cost. We will therefore ensure that you are well informed of any treatment before you decide to go ahead.

What anatomical structures need to be avoided during dental implant placement?
There are no important risk areas in the upper jaw as long as the dental implants remain within the bone that used to support your natural teeth. For patients who are missing any upper back teeth, the region above the roots known as the maxillary sinus should be avoided, and this area will show up on most X-rays.
The most important anatomical structure to avoid in the lower jaw is a nerve known as the inferior dental nerve which runs from the area behind the wisdom teeth through to where your pre-molar teeth are (or once were).

Can dental implants be placed next to natural teeth?
It is standard procedure to place dental implants next to natural teeth. This is considered to be extremely safe unless the natural root happens to be very curved or angled unfavourably in the path of the planned implant, causing potential damage to the root. In most cases this can be avoided by careful planning prior to the surgery.

Is it uncomfortable when the implants are placed?
We place dental implants using the same form of anaesthesia used for routine dentistry as this is very effective and will minimise the discomfort that you will feel. In most cases the surgery entails exposing the bone in the area where the implant and/or bone graft is due to be fitted.

What do I have to do before a sedation?
Prior to your implant surgery, we may ask you not to eat or drink for at least four hours. You will also require an adult to take you home, and you must refrain from operating machinery for up to 36 hours afterwards.

General anaesthetic
In cases where bone needs to be grafted from the patient’s hip to their mouth or where a high number of implants are being placed simultaneously, a general anaesthetic may be required, and this would involve admission to a hospital. For some patients, the amount of bone loss they have suffered after the removal or loss of teeth means that they do not have enough bone left to support a dental implant. Fortunately there are solutions to this problem: sinus augmentation and onlay grafting.
Employing a procedure known as sinus augmentation, it is possible to boost the height of available bone in the upper jaw above the back teeth by creating new bone in the sinus. This technique provides implants for patients who would otherwise be unable to have them in a part of the mouth where teeth are so often missing.

Onlay grafting is a simple concept that involves taking a piece of bone from another part of the body and applying it as an ‘onlay graft’ to a deficient area. Over time, once the bone has successfully joined to the underlying region and has healed and matured, a dental implant can be placed in a more favourable position.

Are there alternatives to using my own bone for grafting?
Yes, there are other sources of bone available which have been prepared for safe use on humans. Just like with your own bone, these materials simply supply you with a scaffold into which the new bone will grow and consolidate so that you are eventually ready for your dental implants to be placed. The time it takes for the new bone to be ready can vary from three months to a year.
There are numerous ways of generating new bone, but one of the most common methods used today is ‘guided tissue regeneration’. It allows slow-moving bone cells time to fill a space by placing a barrier material between them and the fast-moving cells of the soft tissues lining the mouth. Originally the technique involved having to remove the barrier material during a separate surgical stage a few months later, but these days it is more typical to employ a resorbable barrier, which disappears naturally a few months later once it has performed its function.

Does bone grafting affect the length of treatment?
Yes, bone grafting will almost always prolong your treatment as it requires greater skill from the dentist than the dental implant procedure itself – but it will also dramatically improve the outcome of your implants. It can also lead to far more aesthetically pleasing results when used in the front of a patient’s mouth.
There are ways of shortening the length of treatment where bone grafting is used. For instance, a dentist may choose to perform the implant placement with bone grafting and the placement of a barrier membrane simultaneously. However, many surgeons still like to keep the bone grafting as a discrete stage, ensuring that the implants are only fitted once the bone grafting is deemed a success. Whatever approach is used by the dentist to maximise the bone quantity, it is usually well worth the time, energy and expense.

All-on-4 Dental Implants vs Dentures
All-on-4 implants are sometimes mistakenly referred to as ‘All-on-4 dentures’. That's probably because the traditional way to replace a full jaw of teeth is with removable dentures, which consist of a gum-coloured acrylic base and teeth made from resin or porcelain.

Dentures fit over the gums and stay in place either with suction alone or with a dental adhesive. Properly fitting dentures shouldn't require any adhesive.

However, with dentures the supporting bone shrinks over time as there are no teeth for it to hold in place, and this change of shape will cause dentures to become loose. This means they need to be replaced regularly to avoid discomfort and problems when eating. Dentures also increase the rate of bone loss.

Removable dentures are custom-made to precisely fit the patient's mouth. The upper denture covers the roof of the mouth and the lower one is a horseshoe shape to allow room for the tongue. They must be removed for cleaning, usually overnight (hence the cliché of false teeth in a glass on the bedside table).

In contrast, All-on-4 implants are fixed in place and should act just like natural teeth in terms of their appearance, chewing and comfort. There is no risk of them slipping out of place while the wearer is talking or eating.

Dentures offer a clear cost advantage over any kind of implants - but they do need to be replaced, on average once every five years.

When is All-on-4 the best option? 5 factors to consider when creating your dental treatment plan

Dr. John A. Hodges says every case involving dental implants is different and explains why it is critical to assess these five factors before you make a final recommendation for All-on-4 treatment.

Dental implant treatment has guidelines, not rules. It’s one of the reasons why I love dental implants so much. Every case is different. The clinician must have a broad understanding of each patient's unique factors, and then devise the best treatment plan for that individual. Even the most routine-appearing implant case can be impacted by factors such as parafunction, occlusion, periodontal disease, the size of the space to be restored, and the patient’s budget.

It is important for clinicians to assess these factors and listen to their patients' wants prior to offering treatment plans. Although these factors must be considered for fully edentulous patients as well, for purposes of this article I will address cases only for patients who have most of their teeth and who are considering removing them for implant treatment.

Full-arch fixed implant bridges—often referred to as All-on-4 (figure 1)—are taking the dental field by storm. These bridges are usually supported by four to six dental implants, which are fabricated from materials such as acrylic fused to titanium, monolithic zirconia, and porcelain stacked to a cobalt-chromium frame. They are often presented with package pricing, which simplifies case presentation, and some practices promote All-on-4 as the best treatment option for their soon-to-be-edentulous patients.
Thanks to aggressive marketing by companies such as ClearChoice and the extensive information available on the internet, patients are more aware than ever of the benefits of All-on-4 bridges. Five to 10 years ago, most patients were unaware that All-on-4 bridges existed. Today, it is common for patients to specifically request this treatment, but is it always in their best interests? As wonderful as All-on-4 bridges are, when are they a disservice?

It would take a very long article, or more likely a book, to cover everything we should consider when recommending All-on-4 treatment, but these are the five factors that I encounter on a regular basis in my dental implant practice:

Speech issues
Difficulty adapting to bridge thickness
Proprioception
Parafunction
High caries index
Let's take a closer look at each of these aspects and, one by one, see how each might affect treatment considerations.

Speech issues
Speech issues are a major concern for patients. What may seem to us to be a minor speech flaw can become an overwhelming concern for the All-on-4 patient. Implant-supported bridges require both vertical and horizontal bulk for strength. Vertical needs vary with different restorative materials, but most bridges require 15 mm or more of height. As a result, clinicians are forced to replace more than just the volume of the missing teeth in order to achieve these measurements. We must often replace bone and soft-tissue volume, even if it is healthy. The result is that the portion of the bridge adjacent to the soft tissue will be bulkier than the original bone and soft-tissue volume that it replaced (figure 2). Consequently, this can have an impact on speech sounds such as "D," "T," and "N," as the tongue contacts the hard palate lingual to the central incisors to form the sound (figure 3). (1) Likewise, horizontal bulk in the posterior can affect the "S" sound, where the lateral borders of the tongue flare upon making the sound. This can result in slurring. (2)
All-on-4 patients need to be made aware of this compromise. They can often retrain their tongues with time and practice, but it is likely to be a significant source of anxiety for them. If speech is a major concern, clinicians must be prepared to offer alternative treatments, such as a plan that saves healthier teeth and replaces missing teeth with short-span bridges or individual dental implants. If the teeth are terminal, a bridge that only replaces missing tooth structure can be an option. However, this will often require more implants, more bone grafting, and more expensive prosthetics, so patients will need to be prepared for the additional treatment time and cost.

Difficulty adapting to bridge bulk
In addition to bridge bulk affecting speech, there may be another downside. It can be very difficult for patients to adapt psychologically to the different feel of an All-on-4 bridge. They are used to feeling the transition from their soft tissue to their teeth. With the All-on-4 bridge, patients feel real soft tissue and then fake soft tissue (the tissue portion of the bridge) prior to transitioning to the teeth. This can be a big problem for some patients, and sadly they often figure out that it's a problem after surgery when the ridge has been reduced. At that point, nothing can be done to get them back to that natural-feeling transition.

I recommend having detailed conversations about this compromise they will experience prior to finalizing the treatment plan to prevent surprises. I show my patients an All-on-4 model and then a model of a traditional bridge (they’ll get the idea even if it’s tooth-borne) (figures 4 and 5). If the bulk will be an issue, then they should consider options that only replace tooth volume. Due to the reduced bulk of these restorations, both vertically and horizontally, more implants likely will be required to support the bridge to reduce pontic spans and the risk of fracture. Distal cantilevers may need to be shortened and the implants placed more posteriorly, which could result in the need for grafting. Patients may choose the traditional All-on-4 bridge after consultation due to lower cost and reduced treatment time. In any case, patients are more likely to accept any compromises when they have options to choose from and know about any limitations prior to treatment.

Proprioception
This is a big one, and its value is often overlooked when recommending dual-arch treatments. Periodontal mechanoreceptors (PMRs), which are present in the periodontal ligament, make teeth sensitive to low forces (<1 to 4 N). (3-4) A dental implant does not have a periodontal ligament, and it takes approximately 10 times more force to register the same proprioception as a tooth. (5) The proprioception associated with a dental implant is similar to that of a tooth affected by local anesthesia. (6) As a result, patients will have difficulty recognizing premature or excessive occlusal contacts. They may generate excessive biting forces due to the lack of feedback. (6) Patients with dual-arch All-on-4 restorations are more likely to bite excessively than patients with remaining teeth.This can result in fractures of the restoration or bone loss
With teeth, feedback from PMRs assists with refined motor movements, so chewing efficiency is improved. With dual-arch implants, less refined movements result in poorer chewing efficiency and greater stress on the restoration and implants due to angled forces. (7)

My dual-arch All-on-4 patients experience more lost implants during healing, broken temporaries, and tooth popping. Always consider restorative options that save at least a few teeth if both arches are undergoing treatment. If this isn't possible, consider staging treatment by treating the upper arch first while opposing natural teeth for the first few months. You may need to modify the teeth to level the occlusal plane. The proprioception of the lower teeth will assist patients in avoiding excessive biting force and learning to recognize occlusal discrepancies. After a few months, you can treat the lower arch. This will give patients time to develop mechanoreceptors in the orofacial tissues—such as muscles, joints, and periosteum—to assist with feedback. (8) Maxillary implants will also benefit from the extra healing time to increase osseointegration prior to being subjected to the traumatic forces of the opposing implant-supported bridge.

Parafunction
Parafunctional habits—such as bruxism, clenching, and irregular chewing cycles—may impact your treatment decision. As discussed in the last section, teeth can detect forces much more readily than implants. (9) Including a few saved natural teeth in your treatment plan may enable patients to sense the parafunction better (figure 7), which will increase the chances of them modifying the negative behavior.

If it is necessary to replace all of the teeth when parafunction is present, you may want to consider recommending a bar-supported overdenture (figure 8). This is still technically an All-on-4 procedure, but it may be a modification from your patients' visions of fixed bridges. I use bar overdentures for patients who brux aggressively. The overdenture can be removed at night and replaced with a flat-planed night guard that snaps onto the bar for retention. This protects the final prosthesis and reduces strain on the supporting implants and bone.

High caries index
It’s an all-too-common scenario: A patient presents with an upper full denture—maybe even an upper All-on-4—and decayed and missing lower teeth. The patient wants a lower All-on-4 fixed bridge. He has a square-shaped jaw, likes to eat steak, and you suspect he’s a bruxer. Teeth Nos. 18, 19, and 30 are fractured to the gumline, having had previous root canals and crowns. Tooth No. 31 has furcation involvement and is mobile. Teeth Nos. 23 through 26 are covered with calculus and have 7 mm pockets, but teeth Nos. 20 through 22 and Nos. 27 through 29 have minimal bone loss and mobility. Should you save the teeth? What are the risks and benefits?

These are my thoughts: If the bicuspids and canines have minimal mobility and are mostly intact (perhaps one needs a crown and one needs a filling), then I would consider saving them. The risk of developing further periodontal disease or failure from decay is likely less than the risk of fracturing the prosthesis or damaging the implant/bone complex due to overly aggressive chewing or parafunction.

Now, let’s consider the same scenario, except five of the six bicuspids and canines have Class V decay, four have interproximal decay, and the teeth are covered in plaque. The cavities aren’t that big—you can fix them—but how are you feeling about the future prognosis? I’m an optimist, but my optimism has gotten me into trouble in cases such as these. It’s difficult to change a habit. And in this scenario, we are asking the patient to improve his home care, visit the dentist more frequently, and significantly reduce his sugar intake. That’s a lot to ask. The risk of losing these teeth to decay in the not-too-distant future likely exceeds the risk of prosthesis fracture or bone loss. Maybe the best treatment for this patient is an All-on-4 bar overdenture. Aside from the ability to lessen stress, this treatment will accommodate a simplified oral hygiene protocol. You will want to invest plenty of time with this patient prior to the final treatment decision.

Conclusion
We are blessed to have these implant technologies in our armamentarium as options to replace missing teeth. In the past, patients seemed happy just to be able to chew again. They were so appreciative. I used to say that I loved treating full-arch cases, because I was addicted to the hugs. Today I still get hugs, but not nearly as often. Expectations have risen. Patients seem less tolerant of the awkward feel of All-on-4 bridges and of the amount of time the procedure takes. As they become even more educated about their options, I suspect this trend will continue.

This is what I recommend: Offer multiple full-arch restorative options in your practice, but take time to assess the best individual options for your patients. Make sure you educate them about the hurdles they will need to overcome, not just the benefits of the treatment. It is time well spent. You’ll have better treatment outcomes and more satisfied patients. And isn't that what we really want?
